Hi, my name is Jaymes and I need people to check out and do this survey for a report me and my fellow students need to design. The survey is completely confidential and usernames will not be used in reporting these surveys.

This survey is to find out what young people between the ages of 13 and 25 need. It will help me and a group of other students find what we can do to make our area more youth-friendly.

Youth suicide is an important topic for young people and the community. We would like to know your thoughts on youth suicide.
15. A – Many young people experience suicidal thoughts at some time in their life. Do you see youth suicide and self-harm as issues for young people close to you?
B – If so, what do you believe have been the main causes of suicidal thoughts in yourself or people close to you?
C – If you were feeling severely depressed or suicidal, who would you like to be available for you to talk to?
We are interested in your thoughts and experience with alcohol and drug use.
16. A – Have you ever experienced any problems arising from the personal use of alcohol or other drugs?
B – If so, what types of problems have you experienced through the personal use of alcohol and/or other drugs?
C – Have you ever gone to a community agency, school staff member, youth worker or other people to talk about alcohol and/or drug related issues?
